NHSBW is looking to provide a new and innovative service to provide care for those with Long Term Conditions, initially in the high and moderate risk categories. We are looking for a tele-coaching element to form the basis of the service. We would like to take a ?kit light? approach but would expect telehealth technology to deployed for those few patients who may benefit. This service should support vulnerable and high risk patients, intervening before they reach crisis, and should augment and interface with existing NHS services (Community Matrons, Practice and Specialist nurses and GPs). This will enable existing services to focus on the higher risk patients. We are looking for the service to comprise the following components: Recruitment and enrolment of suitable individuals using the PCTs risk stratification tool Accredited Care Groups (ACG). Nurse-led telephone and online support to individuals suffering from LTCs in the moderate and high risk categories. We estimate that this is currently around 19,000 patients. Liaison with, Community Matrons, District Nurses, Practice Nurses and GPs to establish clinical parameters for each patient and escalate should the patient?s condition require it. Provision and monitoring of telehealth equipment to the few patients who would benefit It is envisaged that the suitably qualified telephone coaches would each work with a specific group of practices as a virtual member of the team, carrying a virtual caseload of patients. Objectives of this service: Patients are supported to self-management their condition. They are able to recognise and report deterioration in symptoms and manage their condition with support from their coach A consequent reduction in non elective admissions, outpatient attendances and GP Out-of-hours calls Improved functional status for patients Improved Patient Reported Outcome Measures (PROMS) A contribution to the PCT?s QIPP plan The PCT intends to make an initial investment of #500k with an option to extend the service subject to the objectives being achieved in Phase 1.
